Financial Aid 2009-2010 Forms

Before completing any forms go to Apache Access to view which documents are needed to complete your financial aid file.

You will need to log on to Apache Access and click on the Academic Life tab and then on the ‘Financial Aid Status’ link located in the financial section of the page.

VERIFICATION FORMS

  • Verification Worksheet ~ 2009-2010 This form is used by students who have been chosen for verification. (Do not submit this form unless requested by the Financial Aid Office).

  • Certification of Assets ~ 2009-2010 This form is required for students and/or parents who left the asset information blank on the FAFSA. (Do not submit this form unless requested by the Financial Aid Office).

  • IRS Filing Form ~ 2009-2010 This form is used when the parent(s) and/or students did not, and were not required to file taxes.  (Do not submit this form unless requested by the Financial Aid Office).

  • Low Income Worksheet ~ 2009-2010 This form is used to list all monthly living  expenses and the source of funds by which they were paid. (Do not submit this form unless requested by the Financial Aid Office).

  • Proof of Marital Separation ~ 2009-2010 This form is used to provide proof that you are separated for the purpose of divorce, and that you are not receiving support from your spouse  (other than child support or alimony). (Do not submit this form unless requested by the Financial Aid Office).

  • Student Support Worksheet ~ 2008-2009 This form is used to enable students with low income to prove that they are providing more than 50% support for a dependent . (Do not submit this form unless requested by the Financial Aid Office).

  •  Parent Plus Loan Authorization Form ~ 2009-2010 This form is used by parents to notify us as to how they wish to receive their Parent Plus Loan Funds.


SPECIAL REQUEST FORMS

  • Dependency Override Request ~ 2009-2010 This packet is used by students who do not have contact with their biological parent(s) and would like to be made an independent student based to their extreme circumstance.

  • Information Release Form ~ 2009-2010 This form must be submitted if a student would like for someone, other other than themselves, to have access to their financial aid information.

  • Re-evaluation Request Form ~ 2009-2010 This form can only be request if your current cumulative GPA is a 2.0! In order to request a revaluation a student’s appeal must have been previously denied for either lack of completion of hours and or GPA.

  • Loan Request Form ~ 2008-2009 This form is used to reinstate financial aid awards that have been previously declined.

  • Satisfactory Academic Progress Appeal Cover Sheet Students who did not meet the satisfactory academic progress requirements of their Financial Aid must submit this form along with a letter and documentation explaining the reason for failing to meet satisfactory academic progress.

  •  Special Conditions Packet ~ 2009-2010 This packet is used to request that a change be made by your financial aid administrator to your income that was reported on the FAFSA.  It will include items that will assist our office in evaluating your request for this “special conditions”.

  • Child Request Form ~ 2009-2010 This form may be submitted if you have a child in daycare.  This will allow the financial aid office to increase your Cost of Attendance Budget.  This may not increase your financial aid awards.

FORMS FOR STUDENTS TRANSFERRING FROM TJC

  • Financial Aid Cancellation Form ~ 2009-2010 This form is used to cancel your financial aid award(s) for the academic semester indicated.

CONCURRENT ENROLLMENT FORM

  • Concurrent Enrollment Form  This form is used if you are receiving financial aid at TJC and would like to receive the Pell grant based on enrollment at TJC and another college.
